Grooming Release and Liability Waiver
I understand that grooming carries some inherent risk, especially for pets that are senior, anxious, or in poor coat or skin condition. I confirm that my pet is up to date on vaccinations and has not harmed a person or another animal. I will disclose any health or behavior concerns before the appointment. If my pet becomes ill or is injured and the salon cannot reach me, I authorize the salon to seek veterinary care at my expense. I agree that the salon is not responsible for conditions that surface during or after grooming that result from my pet's preexisting health, coat, or skin condition.
Pet Health Disclosure
Your groomer works hands on with your pet for an extended time. Please tell us about anything that could affect the groom: allergies, skin conditions, seizures, heart conditions, joint pain, recent surgery, fleas or ticks, or anything else your vet is watching. Honest answers keep your pet safe and comfortable.
Matting Acknowledgment
Severe matting pulls on the skin and can hide sores, parasites, and bruising. Dematting a badly matted coat is painful and unsafe, so in those cases the humane choice is to shave the coat short and let it grow back healthy. Shaving a matted coat carries extra risk of nicks and irritation because the mats sit tight against the skin. If we find matting beyond what brushing can safely fix, we will shave the affected areas and tell you exactly what we found.
Emergency Veterinary Authorization
In the unlikely event that my pet needs urgent medical attention while in the salon's care and I cannot be reached at the numbers on file, I authorize the salon to transport my pet to my preferred veterinarian, or the nearest available veterinarian if mine is not available, and to approve reasonable emergency treatment. I agree to be responsible for the resulting veterinary costs.
Photo Permission
I give the salon permission to take photos of my pet during the groom and to share them on its website and social media. The salon will not share my name, address, or phone number with those photos.
